Please read and sign the waiver below before your first class.
RELEASE AND WAIVER OF LIABILITY
In consideration of being allowed to participate in any way in TCB Fight Factory's martial arts programs, related events and activities, I acknowledge, appreciate, and agree that:
1. ASSUMPTION OF RISK
The risk of injury from the activities involved in martial arts training is significant, including the potential for permanent paralysis and death. I knowingly and freely assume all such risks, both known and unknown, even if arising from the negligence of the releases or others, and assume full responsibility for my participation.
2. WAIVER AND RELEASE
I willingly agree to comply with the stated and customary terms and conditions for participation. I hereby release TCB Fight Factory, its owners, coaches, employees, and agents from any and all liability related to my participation in martial arts training, including any injury, accident, illness, or death.
3. MEDICAL CONSENT
I consent to receive medical treatment which may be deemed advisable in the event of injury, accident, or illness during my participation. I understand and agree that I am responsible for my own medical insurance and will maintain coverage during my participation.
4. PHOTO/VIDEO RELEASE
I grant TCB Fight Factory permission to use any photographs, videos, or other recordings of my participation for promotional and marketing purposes without compensation.
5. ACKNOWLEDGMENT
I have read this release and waiver of liability, fully understand its terms, and understand that I am giving up substantial rights, including my right to sue. I acknowledge that I am signing this agreement freely and voluntarily.
